We would like to discuss on the report on “micronuclei in breast aspirates”.[1] Hemalatha et al. concluded “micronuclei scoring can be used as a biomarker on fine-needle aspiration cytology smears of breast carcinoma”.[1] There are some concerns on the scoring system. First, as a cytological investigation, it requires experienced cytologist for scoring.[2] In addition, the problem on interobserver variation can be expected.[3],[4],[5] Second, focusing on the result reported by Hemalatha et al.[1] that the scoring in malignant and nonmalignant lesions could be observed, there are some previous reports with discordant results. Samanta et al. noted that grade 1 ductal carcinoma had the same score as fibroadenoma, and this can be a problem in diagnosis.[6]
