We would like to discuss on the report on “micronuclei in breast aspirates”. Hemalatha et al. concluded “micronuclei scoring can be used as a biomarker on fine-needle aspiration cytology smears of breast carcinoma”. There are some concerns on the scoring system. First, as a cytological investigation, it requires experienced cytologist for scoring. In addition, the problem on interobserver variation can be expected.,, Second, focusing on the result reported by Hemalatha et al. that the scoring in malignant and nonmalignant lesions could be observed, there are some previous reports with discordant results. Samanta et al. noted that grade 1 ductal carcinoma had the same score as fibroadenoma, and this can be a problem in diagnosis.
